We are located at 3230 Pennsylvania Ave, SE, Washington DC 20020, and we serve patients in both DC and MD (Prince George’s County). With our unique care model, we aim to grow rapidly both regionally and nationally.

Essential job duties and responsibilities

We are looking for Nurse Practitioners to join our team on a part- or full time basis. The responsibilities will be, amongst others:

  • Practice primary care through home visits as well as telehealth and in our health center(s) in a way that sets a high standard for other clinicians joining our primary care team. Delivering high quality, patient-centered home visits (housecalls) efficiently will be a core responsibility.

  • Support the Medical Director to develop the care team (Registered Nurses, Licensed Practical Nurses, Medical Assistants, Behavioral Health Workers, Social Workers, and Emergency Medical Technicians) that collectively manages a panel of patients across the different care modalities (office, home, virtual).

  • Appropriately order diagnostic studies, interpret results, and create individualized treatment plans using evidence based medicine.

  • Manage medical and surgical conditions with knowledge of indications, contraindications, and adverse effects of treatment modalities.

  • Counsel and educate patients and families.

  • Perform accurate and timely documentation in the electronic medical record.

  • Be a role model, mentor and coach for all others on the team.

  • Perform other related duties as assigned.

Qualifications

The ideal candidate has a strong affinity or connection with the populations we serve (Medicare, including patients with both Medicare and Medicaid. Key criteria include:

  • Leadership skills, passion for teamwork, ability to coach/mentor team members in a young, dynamic organization.

  • Master's degree and/or equivalent and successful completion of accredited nurse practitioner program (Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner).

  • Unrestricted license to practice as a Nurse Practitioner in DC and Maryland.

  • CDS and DEA for both DC and Maryland preferred

  • Minimum of 2 years work experience as a Nurse Practitioner.

  • Relevant experience (working with elderly patients, lower-income patient and/or palliative care) preferred

  • Enthusiastic to work in a performance-driven organization (data-driven, focused on continuous improvement of patient experience, care quality, and outcomes)

  • Experience with Electronic Medical Records and office software.

  • Active driver’s license and reliable transportation
